The FDA has issued a potential‑high‑risk alert for certain Commander M delivery systems that are used to implant Edwards Sapien M3 mitral valves during transcatheter mitral valve replacement. The alert follows three reports of balloons bursting during inflation, a problem that can lead to embolism and make it impossible to retrieve the device through the guide sheath. Because the patients who receive this therapy are usually unsuitable for open‑heart surgery, a failure to retrieve the valve or a subsequent surgical intervention carries a high risk of death. Edwards reported one patient death and one serious injury linked to the issue, and the FDA has advised that affected product lots be removed from use immediately.

The Sapien M3 transcatheter mitral valve replacement system received FDA clearance in December 2025 and is intended for patients with moderate‑to‑severe mitral regurgitation who cannot undergo surgery or edge‑to‑edge repair. The alert does not involve the valve itself; only the Commander M delivery system is affected. Four specific lot numbers – 67718561, 67804014, 67887043, and 67887044 – are listed for removal. Edwards had previously recalled its Sapien 3 Ultra delivery system in 2019 for a balloon‑burst problem involving 2,579 units. The FDA is still investigating the severity of this early alert and has not yet determined how many patients may have been exposed.
